This form is used to monitor unsportsmanlike behavior from our member schools. Any conduct by coaches, players, or fans that is unsportsmanlike—but does not rise to the level of a disqualification—should be reported.
It is strongly encouraged that this report be used in all applicable cases.
Using this report helps ensure that school administrators become aware of unacceptable behavior that they may not otherwise notice.
The form is a simple drop-down document that takes only a few minutes to complete, and it also includes a section for additional explanation.
Please take the time to report these issues. Doing so will help improve the environment for the next crew.
